Certificate Issuance Notice
- Applications via telephone, email or fax are not accepted.
- Certificates will be issued under the name used while enrolled at the school.
- If the documents are incomplete or the fee is insufficient, the certificate cannot be issued.
- If you are a graduate of Osaka Sangyo University Dental Hygiene College, please apply via the application page for certificates, etc. for Osaka Sangyo University University Dental Hygiene College on the School Juridical Person Osaka Sangyo University website.
- If you have any questions regarding the "Certificate of Academic Ability", please contact Center of Teacher Education.

2022年7月1日より証明書オンライン申込システムを導入しました。 大阪産業大学卒業生、大学院修了生、大阪産業大学短期大学部卒業生、ならびに除籍?退学された方が対象です。 【在学生?休学中の方】は利用できません。

上記以外の証明書が必要な方は、郵送または窓口でお申し込みください。 （やむを得ず代理人が申し込みをする場合は、窓口または郵送でお申し込みください。証明書オンライン申込システムはご利用いただけません。） 「学力に関する証明書」（教職用）につきましては、教職教育センターにお問い合わせください。
How to use the online certificate application system
- In addition to the certificate issuance fee (see separate table) and the certificate return fee (including express delivery fee), a system usage fee of 400 yen is required each time.
- You can apply for up to 20 tickets per application.
- Payment methods are credit card*1 convenience store Pay-easy.
*1 You or a relative must have a credit card (VISA, MasterCard, JCB, American Express). If you are applying from outside Japan, payment must be by credit card.
- You will need to upload an image of a government-issued identification document*2 that can verify your identity via the internet.
- Once payment has been completed, the certificate issuance fee, certificate return fee (including express delivery fee), and system usage fee will not be refunded under any circumstances.
- Receipts cannot be issued.
*2 Official identification: A valid driver's license issued in Japan (both sides), passport (photo page and holder name section), or Japanese-issued health insurance card. Please contact us in advance if you are using any other form of identification.
Each certificate will be sealed and sent in a return envelope. Japanese certificates will be sent within 5 business days of payment completion. English certificates will be sent approximately 10 business days of payment completion. Please apply with plenty of time. The office is closed and does not issue certificates on Sundays, national holidays, summer and winter holidays, and our founding anniversary (November 1st). Certificates will be created and sent in the order they are created once operations resume.
